Getting started
When you're ready to request services, simply click the “Get Started” button.
- New Clients: If you’re a new client, please create an account in our client portal.

This account will also provide access to all your intake paperwork, a billing summary once sessions begin, and a complete log of all your sessions.

- Returning Clients: If you’re already a client, log in using your existing portal credentials.

Once you’re logged into your account:
New Clients: You will find the Intake Bundle, a Comprehensive Clinical Assessment packet, and the Service Request form. Please complete the Service Request form so our team can assist you in pairing with a provider.
If you do not complete the service request form our team will follow up with you as its essential for starting care.
After submitting your request, you can begin your intake forms to ensure everything is ready for your intake session, helping to prevent any delays in service.
Current or Returning Clients: You will see one form in your portal: the Service Request form. Please complete this form to indicate whether you’d like to add an additional provider, resume care with a former provider, or find a new provider.
If you are having issues getting back into your account please do not hesitate to reach out.
Once you submit your Service Request, our referral team will work to pair you with a provider based on your insurance, preferences, and whether you prefer virtual or in-office sessions. If we need any additional information to proceed, our team will be in touch.
Please note that while we strive to accommodate all preferences, there may be instances where we cannot meet certain requests. If that happens, we will be sure to reach out to you.
Once a provider has been identified, they will receive your contact information and will reach out to you within 24 business hours to schedule an intake session. At that time, you will also receive a welcome email from our Intake team, and your provider's Personal Disclosure form. Please ensure you have all your paperwork ready before your intake session.
