Insurance Confirmation


Thank you for providing your insurance information.

It is important that you know that the verification is not a guarantee.

We do our best to understand your insurance benefits so that we are able to collect payments at the time of service based on your reported deductible or copay. Payments are due at the time of service. Once the claims have been processed and it is determined that the member owes more/less to the provider we will make that adjustment and that will reflect on the end of the month statement if there is a balance/credit due.

You should receive an email confirmation shortly with a copy of the information that you submitted on the form.


Here are the remaining steps to get started

  1. Fill out the intake paperwork and bring this form with you to your first session,  here is a link for the intake paperwork, please use the latest Adobe Reader/Viewer
    • Please Download this fillable PDF, save, Rename, and complete
  2. Choose a counselor.

When you arrive, our office is located on the first floor. Entering through the main entrance, facing SE Tech Center Pl, take a right turn, we are the 3rd door on the right, Suite 180.
